Pursuant to § subsection 2 of Act No. 101/2000 Coll. on Personal Data Protection, as amended (hereinafter referred to as “the Act”) and Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons concerning the processing of personal data and on the free movement of such data I hereby consent that employment agency AGENTURA PLUS s.r.o., Evropská 690/138, 160 00 Praha 6, IČ 25759973, registered in the Commercial Register kept at the Municipal Court in Prague, section. C, file 67745, and other companies that are the members of this group (hereinafter referred to as “the administrator”) can collect, store and process my personal data and I also agree that my personal data can be processed and used by employees of the administrator or contractual partners of the administrator.

This consent is given for an indefinite period of time.

I hereby consent that the administrator can use and process my personal data to list me in the database of job applicants and to send me job offers from the administrator´s customers who are my potential employers, not to send me only job offers that are publicly advertised.

In case that an employment contract is concluded (the employment agency is your employer) your personal data will be processed under GDPR. Your name, address, date of birth, personal identification number, salary, etc. are the data which we are obliged to record, process and archive for reasons imposed by one of the following laws (Act No. 586/1992 Coll. On Income Tax, Act No. 582/1991 Coll. on the Organization and Implementation of Social Security, Act No. 262/2006 Coll., Labor Code, Employment Act No. 435/2004 Coll., etc.)

I hereby declare that I have been informed in accordance with § 11 of the Act that I can revoke my consent to the personal data processing at any time unless my employer is obliged to process and store my personal data for legal reasons. In such a case, I can revoke this consent only after the date stipulated by law. I am fully aware of my rights under the provisions of § 12 and § 21 of the Act and I declare that I have provided all my personal data voluntarily.

The administrator undertakes to collect the personal data to the extent that is necessary for him to fulfill the above purpose and process them only in accordance with the purpose for which they were collected.

The administrator hereby declares that he will process the data in both computer and written forms.

The data subject hereby confirms that this consent is a free and informed expression of the data subject´s free will and that he/she agrees to the processing of his/her personal data.
